Finding the right help for seniors during life's transitions can be a challenging task. If your family has questions about what to do to meet the needs of a loved one, we can help.
At Elder-Care-Services San Diego our goal is to help our clients achieve the best quality of life possible. We understand that one solution does not fit every situation so we take the time to listen and learn about each client's needs and preferences. This enables us to tailor a care program for the elderly that maximizes the client's independence and dignity.
Finding the right professional can be challenging - you may find that an internet search will list professional services that sound similar but have very different names - elder care adviser, senior care services, case manager or care manager, services to seniors with dementia, Alzheimer's or other issues related to aging, geriatric care managers - the overlap in the range of services out there can be overwhelming. So where do you start?
We provide a broad range of services to the elderly that includes assessments, crisis intervention and development of a personalized Plan of Care specific to each client's unique needs. We work with the client and family to find the very best and most appropriate care plan, whether that is remaining at home or outside placement. Our goal is to make every transition as comfortable as possible for the client and family and is based upon mutual trust, support, listening, respect and confidentiality.
We can arrange for a variety of services tailored to your family's needs that includes: visitation and monitoring, reporting, referrals, visits from registered nurses, physical therapists, social workers and home care aides.
We have found that many families of seniors in need are still in the workforce or have young children at home and may find it difficult to do everything that needs to be done to feel comfortable with their loved one's care.
Having someone you trust who is there to supplement the family's efforts can be invaluable.
Our Services include:
- Assessment and review - meeting with the family to help determine what you need, go over options and determine the best way to proceed
- Crisis intervention - working with the client and family to come up with alternatives that suit your family based upon your unique circumstances
- Development of a Plan of Care that will set goals and specific tasks to oversee the care needs of the client including on-going care management
- Monitoring and visitation whether at home or in a facility, along with regular reporting to the family - this can be essential to a sense of comfort in overseeing care
- Referrals to other professionals, services, community resources and entitlement programs
- Assistance with home management, bill paying and estate management including cleaning, interior and exterior maintenance, pest control and emergency services
- Supervision of activities of daily living including: hygiene assistance, laundry, shopping, meals, cleaning, transportation, financial planning
- Medical appointments - including transportation, follow-up, scheduling appointments, health advocacy and home health aides
- Relocation support services - we will provide professionals to support families during transitions from home to another location or facility
- Home safety - including a home safety assessment followed by recommendations and support to make safety modifications - from a team experienced with ADA requirements and guidelines
- Long distance support - we specialize in helping families who have loved ones at a distance - we will provide support services, conduct regular checkups and reports to the family
